NEW ENROLLMENTS FOR THE 2024/25 SCHOOL YEAR WILL BE ACCEPTED STARTING MARCH 4, 2024.

Parents/guardians are required to complete the Pre-Enrollment application to register a new student to the Moreno Valley Unified School District. Please use this application to provide information about your household and your student(s).

Submitting this application will begin the registration process for MVUSD.

All documentation must be either uploaded to with this application or brought to our Welcome and Registration Center to complete the registration process. Once submitted, the application will be reviewed and you will be contacted via email.

If the following documents are not uploaded with the application, you may bring them to:

You must upload through this application or bring in the following:

Required for Each Family:

Picture ID of Parent/Legal Guardian
Proof of Residency (One of the following dated within 60 days and must show parent/guardians name)

Gas, Water or Electric Bill (No telephone or cable will be accepted)
Lease agreement or closing escrow papers
Mortgage Statement
Bank Statement
DPSS Statement

Required for Each Student:

Birth Date Verification (One of the following)

Original Birth Certificate
Adoption Papers
Baptismal Record
Hospital Birth Record
Passport

Immunization Record

Required for TK and Kindergarten and First Grade:

Signature of Parent/Legal Guardian on Kindergarten Pre-Enrollment Form (TK and Kindergarten Only)
Physical Exam completed by doctor(required for Kindergarten or First Grade within 6 months prior to first day of attendance)This form is available through your family doctor or here: State of California Report of Health Exam Form
